Online Boiler Appointment Request Form
This form is used to request Boiler Division inspections for sign offs on new boiler installations, replacements or minor alterations/repairs. The unit that issued the permit can verify if a job specifically requires a Boiler Division sign off. Each sign-off inspection request will receive a confirmation with the date and time of the appointment, as well as any specific instructions the installer must follow the day of the inspection. Applicants with questions may call the Boiler Appointment Clerk at (212) 566-4916 or (212) 566-3357.

CCD1: Construction Code Determination
The "Construction Code Determination" form is used to request a determination (formerly known as reconsideration, pre-consideration, or interpretation) for all issues that are non-zoning related, for a job or planned job, from the Department. Upon submission, Department staff will review the request and issue a decision. Note that the Department will only review determination requests submitted on this form, and no longer accept them on the "Pre-Consideration and Reconsideration Application" (BC1) or "Additional Information" (AI1) form.

ZRD1: Zoning Resolution Determination
The "Zoning Resolution Determination" form is used to request a zoning determination (formerly known as reconsideration, pre-consideration, or interpretation), for a job or planned job, from the Department. Upon submission, Department staff will review the request and issue a decision. Note that the Department will only review determination requests submitted on this form, and no longer accept them on the "Pre-Consideration and Reconsideration Application" (BC1) or "Additional Information" (AI1) form.

HRT-1 High Rise Initiative - Program Request
Used by public to request participation in the High Rise Initiative program, available to New Building and Alteration 1 Enlargement high rise projects. Request form will be reviewed. If the applicant is eligible to join the program, a 1-hour appointment will be scheduled. Please review the Administrative Items Check List and the Drawing Submission Guideline for the list of documents the applicant has to provide during the meeting.
