Online Boiler Appointment Request Form
This form is used to request Boiler Division inspections for sign offs on new boiler installations, replacements or minor alterations/repairs. The unit that issued the permit can verify if a job specifically requires a Boiler Division sign off. Each sign-off inspection request will receive a confirmation with the date and time of the appointment, as well as any specific instructions the installer must follow the day of the inspection. BO9 DOB Boiler Inspection Report/ DEP Boiler Renewal Request
To be filed at the
Department of Buildings (DOB) in order to fulfill the DOB annual boiler
inspection requirement (replaces OP1-LP & OP1-HP). May also be used to
fulfill the triennial renewal of boiler registration/certificate to
operate as required by the Department of Environmental Protection
(DEP).

BO13 Boiler Inspection Defect Report and Affirmation of Correction
This form provides boiler owners with details of defects found on a boiler during the annual low pressure boiler inspection in accordance with §28-303.2 and department rules. Boiler owners must submit this form to the Department of Buildings within 45 days of an annual inspection, as recorded on the BO9, when defects are noted. Prior to submitting the form to DOB, the boiler owner must have the form completed by an authorized boiler inspector, certifying that the defects have been corrected.
