Elevator Inspection/Test 2009 Category 1
Extension Form
Executive Order 9/09 extends the elevator Category 1 inspection/test cycle for Calendar Year 2009 to April 30, 2010. Building owners seeking to perform the Category 1 inspection/test after December 31, 2009 must submit the Elevator Inspection/Test 2009 Category 1 Extension Form to request an extension.
