ED16A Electrical Permit Application
This form should ONLY be filed by Licensed Electricians. It can be used for all types of electrical work, including the removal of electrical violations to the owner. Approved applications will produce a work permit, which is required to be posted at the job site by the Licensee. Upon completion of the job, final inspection is required by a DOB inspector and a signoff is issued by the Department prior to the use of the installed electrical work. The Licensee is required to call the borough office where the work is completed to schedule an inspection.

BEC43 Electrical Self-Certification Affidavit
This form allows a licensed electrical contractor to self certify an "A" violation. "A" violations are issued on jobs that are completed and have been inspected and violations have been issued to the contractor.

ED76: Electrical Code Interpretation Request Form
This form must be submitted to the Electrical Code Revision and Interpretation Committee (E.C.R.I.C.) to request an interpretation and/or clarification of Electrical Code Technical Standards. The form should be submitted two weeks before the E.C.R.I.C. meeting date in order to be considered for that meeting.

ED77 Electrical Code Amendment Proposal Form -
2010 Electrical Code Revision Cycle
To ensure that New York City is on the cutting edge of technology and electrical engineering, every three years the Buildings Department adopts the most current National Electrical Code (NEC) with New York City amendments. The technical review of the 2008 NEC is now under way and we are soliciting proposals. Submit the proposal form by July 31, 2008 to suggest a code change for the 2010 NYC Electrical Code.
